Resident-based supportive services that include comprehensive case management are the linchpin of the organization's mission to help each individual achieve his or her full potential. The organization offers some of the most comprehensive resident-based programs of any organization in the City of Philadelphia. Transitional Housing ProgramThe organization operates this highly successful program for the City of Philadelphia. The program focuses on helping homeless families transition from temporary or transient housing to permanent, safe and affordable housing. The program provides intensive case management to 115 families each year. The case management includes the development of an Individual Service Plan (ISP) that sets short and long-term self-sufficiency goals for each family. FRP staff develop these plans and assist each family with locating safe, decent and affordable housing, finding the supportive services they need to help them to achieve their full potential, and tracking and monitoring the families as they travel along the continuum to self-sufficiency. |
